I am the owner of Lori G. Eisen Occupational Therapy Services, and provide occupational therapy assessment and treatment, and case management and life care planning services for individuals that have sustained catastrophic and non-catastrophic injuries within both the private sector (e.g., motor vehicle accidents [SABS & Tort], medical malpractice, long-term disability, accidental shootings, slip-and-falls) and general community (e.g., chronic health conditions, aging). I am a Registered Occupational Therapist and Canadian Certified Life Care Planner, and have been working with individuals and their families offering my rehabilitation services for over 22 years. I specialize in the physical, functional, cognitive and psychosocial evaluation of individuals who are unable to perform essential tasks of daily living or work due to illness and/or injury. My practice includes individuals who have sustained spinal cord injury, traumatic brain injury, amputation, complex orthopedic injury, and multiple traumas. My Life Care Plans/Future Cost of Care Reports (i.e., to assist in the management and determination of current and future care needs) and Loss of Service Reports (i.e., for families of individuals who are deceased as a result of an accident or negligence) are used in rehabilitation planning, mediation, arbitration, settlement discussions, and litigation. I am a Certified Catastrophic Impairment Assessor based on the AMA Guides to the Evaluation of Permanent Impairment, 4th Edition.
